Duplicate License

Required Documents for Duplicate License:

1. Required Duplicate Application Form.
2. A copy of N.I.C
3. 2 Attested fresh Photographs (Passport size).
4. Report from Computer section for Computerized License or A certificate issued by relevant Post Office which expresses the expiry date of license.
5. Ticket will be pasted according to the schedule.
